What Is Zepbound?

Zepbound is the brand name for tirzepatide approved by the FDA in 2023 for chronic weight management in adults with obesity (BMI ≥30) or overweight (BMI ≥27) with a weight-related condition, and in 2024 for moderate-to-severe obstructive sleep apnea in adults with obesity. It is the same dual GIP/GLP-1 receptor agonist as Mounjaro, made by Eli Lilly, and is currently the most effective FDA-approved medication for weight loss.
How Zepbound works
Zepbound activates two gut hormone pathways at once — GLP-1 and GIP. GLP-1 suppresses appetite, slows gastric emptying, and enhances glucose-dependent insulin release. GIP further enhances insulin secretion and appears to improve how adipose tissue handles fat. The dual action produces stronger appetite suppression, greater insulin sensitivity, and meaningfully larger weight loss than single-agonist GLP-1s in head-to-head trials.
Clinical results and dosing
In the SURMOUNT-1 trial of adults without diabetes, Zepbound produced average weight loss of 15% at 5 mg, 19.5% at 10 mg, and 20.9% at 15 mg over 72 weeks — versus 3.1% with placebo. Roughly one-third of patients lost 25% or more. In SURMOUNT-OSA, Zepbound reduced sleep apnea events by 25-29 per hour and led to FDA approval for OSA in 2024. Dosing starts at 2.5 mg weekly and titrates monthly through 5, 7.5, 10, 12.5, and up to 15 mg maintenance.
Side effects and long-term considerations
Side effects are typical of the GLP-1 class: nausea, vomiting, diarrhea, constipation, and reduced appetite, mostly mild and dose-titration related. Serious but rare risks include pancreatitis, gallbladder disease, and the boxed warning for thyroid C-cell tumors. Preserving lean mass during rapid weight loss requires protein intake of 0.7-1.0 g per pound of goal body weight and resistance training 2-3 times weekly. As with all obesity medications, stopping the drug typically leads to substantial weight regain because the underlying biology persists.
