What Is Hypothyroidism and Weight?

Hypothyroidism is a condition in which the thyroid gland produces insufficient thyroid hormone, slowing metabolism and contributing to weight gain, fatigue, cold intolerance, and other symptoms. While hypothyroidism rarely causes dramatic weight gain on its own (typically 5–20 pounds), untreated thyroid dysfunction makes weight loss significantly more difficult.
How thyroid affects weight
Thyroid hormones (T4 and T3) regulate basal metabolic rate. When levels fall, resting calorie burn decreases, fluid retention increases, and energy for activity drops. Even subclinical hypothyroidism can affect weight.
Diagnosis
A full thyroid panel includes TSH, free T4, free T3, reverse T3, and thyroid antibodies (TPO, TG). Hashimoto's thyroiditis — an autoimmune cause — is the most common reason for hypothyroidism in developed countries.
Treatment and weight loss
Standard treatment is levothyroxine (synthetic T4); some patients respond better to combination T4/T3 therapy. Once thyroid levels normalize, weight loss becomes possible through standard strategies — though metabolic recovery can take months.
